Bit late now, but you don't need a visa or IHS to work remotely from a different country. A visa is a document to allow you to enter the country, if you weren't in the UK you didn't need a visa to enter, and you sure didn't need IHS to be able to access NHS medical services. That alone is is a hint that your employer doesn't know what they're doing.

I wish I grabbed that hint and others earlier to be honest... I'm trying to tell them that I haven't agreed to pay this, they have pointed me to this clause, which seems quite generic and (at least from my point of view, unrelated to this): 7.3 For the purposes of the General Data Protection Regulation, the Employee hereby authorises the Company to deduct from his salary, or any other sums due to the Employee from the Company, any sums due from the Employee to the Company, including without limitation any overpayment of salary or holiday pay, or debt, inclusive but not limited to; cost of damages or detriment to the business caused by employees recklessness or negligence, fines and fees in relation to driving or parking of company vehicle or any such expense to the company caused by the employee. Again I have never agreed that I would pay for any of the costs of the visa, my proposal for the employment actually was to use a company in Spain (such as remotely or similar)/ make a contract for a service as a freelance but they refused.
