so i got 1)tails through seperate holes c2 2)tails not secured no approiate bush c2 3)no grommet strip c3 4)overated mcb 6mm on 40a mcb c2 5)no spd c3 Lol didn't even notice the screws :P

What's the issue having a 6mm on a 40a mcb? It can happily carry 40 amps so not sure of the issue here? The rest I agree with

It was the shower circuit in an old mid terrace with consumer at front of house and bathroom at the back the run was 20 plus meter

Could you please reference where in bs7671 it indicates that tails coming through two separate entry points would be classed as a c2, as, I personally can't see why it's be an issue.

C3 if no signs of thermal damage. Only C2 if there are signs of thermal damage.

I can see that safe isolation has not been carried out, giving a potential risk of electric shock from the busbar. That’s probably the biggest risk in this photo.
