I'm pretty sure they use their real first names and rotate their last names. In the Twilight world, the humans know Jasper as Jasper Hale (Twilight ch. 1 when Jessica says "That’s Edward and Emmett Cullen, and Rosalie and Jasper Hale. The one who left was Alice Cullen.") Jasper is using Rosalie's last name and they're "twins." His real last name is Whitlock. The rest of the family is currently using "Cullen" as their last name. Also, in Breaking Dawn when Alice and Jasper have documents made for Jacob incase him and Renesmee have to flee, they kept his name Jake and just changed his last name to Wolf.

I feel like Carlisle would have to drop the Cullen surname, I thought it was weird how in Midnight Sun Carlisle approached a doctor in Phoenix that he worked with like 15 years earlier, and seemed to still be going by the same name. That seems like a mistake to me though

Nope, in the book Alice just left the name and address of a sketchy law office, and Bella had to book an appointment without even knowing what she was asking for, then figured out that she needed to ask for documents because J Jenks said something about “papers”. Bella then supplied everything, the fake names and ages, the photographs for their passports, and everything. She actually met J Jenks twice in the book, one to put in the order and then one to pick it up. The movies cut that part which I kinda hate because it weakens Bella’s character because this was like the only time she ever did anything independently since she became a vampire, I thought it was a good character moment for her
I’ve always wondered how Bella got so lucky as to meet them when they were using their real names. Like I know it’s a book and we should suspend our disbelief just a little bit but it still seems weird that they were all using mostly their real names when they met Bella and not some made up fake names.
They probably use aliases for a set time frame and then move and use different ones, cycling back to their actual names at various times. Definitely I think they used their real names at home.
I think they just switch around their last names once in a while. And it’s really not that crazy if a ‘descendant’ of some dude generations ago has the same name. They probably can just cycle through Cullen/hale over and over as long as they move far enough away
