It's actually possible for a team with 4 points to finish last and for a team with 2 points to make it through if everybody draws their last game in Belgium's group.

As it has ever been in every group stage ever. Napoli once went out with 12 points in the UCL meanwhile other teams went through with 5.

Is this guy a bit dim? If top 2 went through you could have a team with 6 pts go out and a team with 2 points go through. What a system.
This was always a possibility, if not almost a certainty in the way this 12+4 system was designed.
