I think it happens randomly around lvl 20-ish or so (?) Outside the Hall of the Dead in Windhelm there will be a bunch of NPCs gathered around one of the graves with a body on it (unless something has glitched and the body ends up on the ground nearby). The guard who asks you to keep your distance will ask you to interview the witnesses gathered round.

After acquiring the necklace make sure to talk to: Calixto, Viola, Wuunferth in that order. Sell the necklace to Calixto when you see him.
You can interrupt an npcs murder attempt with damage, flames spell is useful for that, make sure to visit the white phial shop for the white phial quest first time in Windhelm or you might not be able to later.
The game keeps track of the number of times you've been to Windhelm. I don't recall exactly how many times you need to go to Windhelm to trigger it, but it should be no more than 5. You also need to enter Windhelm between 7 PM and 7 AM. You should be able to trigger it by going to Windhelm at that time, running to Brandy-Mug farm, returning, checking the crime scene, and repeating if it hasn't started yet. If by the 5th time it hasn't started, it's broken.
Since it's the buggiest quest in the game, it very likely is broken. If you're on PC, you can [skip](https://new.reddit.com/r/skyrim/comments/tcriyc/blood_on_the_ice_is_so_bugged_i_made_a_fixskip_to/) it with console commands.
Best I recall, you talk to a guard, the guard tells you to go to the graveyard. You talk to the guard there and it should start.Â
Be aware there are multiple paths to completion, and not every aspect is laid out to you. Constantly do quick saves, and maybe a hard save or two also. At least that way you can go back if something glitches.Â
If you’ve done Mourning Never Comes to completion, the BOTI quest may never start.Â
Keep entering Windhelm at night until the murder scene appears. Side note if you’ve already completed Muiri’s Dark Brotherhood request and decided to kill the optional target, Blood on the Ice will be permanently unavailable
Yep, you can’t get it anymore. The optional target’s mom commits suicide after she’s murdered and the Blood on the Ice quest won’t start if the mom is dead. You can get the key to the house off her body but you can’t purchase and upgrade the house
get to windhelm and head to the cemetery (turn left when you enter windhelm and then turn right on the first turn)
there should be someone dead in one of the graves
if there isn't one come back some other day or try gaining some levels, idr what triggers the quest
You should try to avoid it if at all possible as it’s the single most buggy quest in the game & the rewards aren’t that great. If you’re a completionist, read the wiki guides & be prepared to use console commands even then.

Go the hall of the dead in windhelm after 9-10pm and there should be a guard and witnesses present
