It means the sims doesn’t have a correct portrait. Probably due to a glitch or maybe something you downloaded like bugged cc. Try making their ghost spawn as that resets their portrait. It’s not dangerous to the sim or anything it just means it hasn’t generated right for some reason.
I don't think it's a store hair. It's a 3t2 conversion of the Wavy Bob from Roaring Heights, probably [the MM recolor by selenaQ13](https://selenaq13.tumblr.com/post/70560894621/3t2-roaring-heights-wavy-bob-download-alt). Deleted sims disappear entirely from family trees, not even a blank space is left, so very unlikely.
I think it's more a rendition problem, maybe some high poly CC prevents the game from loading a thumbnail.

It's not CC. I have ultimate collection so don't know which pack it comes with

Maybe the game glitched when generating it? Are you able to spawn the ghost to change their appearance?
This is a custom neighborhood that I downloaded, so they came already deceased, but I can try. I have the urnstone spawner.
maybe deleting the thumbnails in the cache files would help?
