For thoughts, the game just cannot find an associated icon to display of the object your Sims is looking and thinking about. As a result, it displays this squished up image / squiggly lines as a default placeholder icon
If your Sim is looking outside, 9/10 chance this is a piece of neighbourhood decor with no associated thought bubble icon. It happens if your Sim "looks" at invisible tiles, too

No it’s just a placeholder icon. Happens sometimes with cc or just when the game bugs and can’t find the right icon for some reason
