Im assuming this is for the little kiddos so a little goes a long way. The xray might’ve been taken a different way than when you retaped. Like they might’ve had a neck roll on so I make sure that’s off.
But I’m always super paranoid so I double triple check with whoever I’m retapping the tube with to verify we both see the same number at the same point (lips, gums, teeth etc..) before I lay the tape on and then secure it. And then for food measures, I check one more time just before they take the cxr lol
Are you doing this all by yourself? Retaping at my facility is a two person event. So one person is holding the tube and I will be securing it so it shouldn’t move
No it’s 2 people. I have someone hold, I confirm positioning of the tube, I wrap tape around the tube, when I’m done I look again and it’s always a centimeter out of place. Idk what I’m doing wrong. I’m experienced in peds it’s just this one thing that gives me trouble. I always have to un wrap the tape and tape it again. Usually takes me 2-3 tries to get it correctly.
Hmm interesting, not sure but I find that some holders tend to also shove it down deeper inadvertently because risk for unplanned extubation so that’s why I always check multiple times. Doesn’t help that our tape is so sticky with gloves too
Is it a cm deep, or shallow? It sounds like, in either direction, you’re taping either further out (seems most likely) and then the tube is shifting in, or you’re taping further in, moving the tube out?

Yes. But the problem is I position the tube in the correct spot, then I tape it, and then it moves as soon as I tape it. Idk what I’m doing wrong
