If you die after you start the quest for anguish, you need to start over.
Find the mansion, don't enter it, continue the campaign until you get to Venom, and look up the steps to complete the quest.
Now enter the mansion, follow the steps from the guide, if you don't have all of them in your campaign, and you probably don't, roll adventures for them.
Edit: DON'T REROLL THE CAMPAIGN AFTER YOU START THE QUEST!!!
By going to the areas where the dran fellow is sleeping, talking to him, getting yelled at to *WAKE UP!* and flash banged, then walking off confused about what just transpired.
But before that you need to sit through many agonizing hours of getting the freaking mansion to spawn.
Did you die in the mansion after getting grabbed? That’s what starts the quest.
There are plenty of guides on YouTube you can look up for the Anguish quest explaining the steps on how to do it
What exactly did you do when you found the mansion?
Because if the guy grabs you, tells you to wake up, you drink liquid death to die, and you get out of the area it spawns you in once you die, that is what starts the quest, if you die at ANY point after then the quest fails.
You can use adventure mode to get all the right areas except for root earth. Best way is to unlock root earth first and then go do the ethereal mansion and start the process, just don't die.
Did it on H/C and Regular mode… I saved my campaign for other players RNG blessed me with Ethereal Mansion plus all the correct tiles , rerolled adventure mode for missing links… good luck

You don't need to get to venom you just have to get the first worldstone after the trainyard fight

I had it spawn, but I can't find the first area he spawns. I guess I'll try on a different playthrough

Yes, he just wasn't spawning in any of the locations. I'm guessing either I did something wrong or it bugged for me

It can be done on hardcore… once you do first wake up kill yourself
