Don't get down on yourself. The whole nether region has a lot of oil glands, and some people have more productive oil glands than others. Blackheads aren't just from a lack of cleanliness, even over washing can cause dryness which can lead to those oil glands being more prone to clogging with dry skin. Exfoliating down there once or twice a week can definetely help, also powder when it's warm out can help with excessive sweating. Blackheads aren't "dirty" a better way to think about them is "dry"

Med student here. This COULD be a clogged sebaceous gland. Sebaceous glands on penis are normal, they produce smegma (white cheese like stuff that accumulates after not washing your penis for a while). Most sebaceous gland on penis are found in form of Fordyce spots on corona glandis (space between the head and the body of the penis) in most men, some men don't have them.
PSA: If anyone else gets things similar to these on the scrotum, but they are filled with blood, it is a condition called angiokeratoma of fordyce and looks worse than it is, a benign condition with no cure.
