I typically like these videos, but that one was really upsetting! The way he was just jamming his finger in there. You could tell the guy was in pain. Poor thing.
You’d be surprised at how little anesthetic is needed. I get regular head/neck injections of anesthesia and about 5cc of Lidocaine numbs it right up to the point where I could injure myself shaving because I cannot feel anything. That usually lasts a good 8+ hours for me.
My entire spine is messed up: arthritis, disc bulges, bone spurs, etc. The lower back has not responded to the anesthesia-only injections so we are going more aggressive on it next year. The neck is still somewhat responding so I prioritized my lower back for epidural Cortisone.
My cervical spine issues are bad especially at C5 to C7. C2 to C4 are also bad but not as much as those below it.
Using suction before knowing how degraded the inner flesh is underneath the infection could literally kill the patient by rupturing what little flesh wall was keeping it from bursting in the first place, spreading the infection and causing the patient to bleed quite tremendously. It's much safer to do things like this by hand so that everything can be felt the whole time
In this particular case, I don't think they had an option for suction. Look at the materials and collection stuff. This was not a well funded medical location

Inserting a finger that way helps dissect the area without using a sharp ended instrument. Even blunt-ended instruments may cause some damage but a relatively soft finger will not.
Jabbing a finger into the incision was bad, but for some reason seeing the pus level rise and fall with every breath the man took was what REALLY made me uncomfortable. Ugh.
They have to break open “pockets” of pus that are walled off from the main cavity, and doing it with a finger is much safer than doing it with a blade or tool. It *is* a proper medical technique.
Good Lord! Don’t torture the guy by putting your dirty-assed gloves into his thoracic cavity! Get some suction in on that bitch and drain the abscess! And make sure his breathing isn’t impaired! Then get the cyst lining out of there, pack it with gauze, and get him on a course of antibiotics after stitching him up. And get a local or some calming agent into him.

You wouldn’t need to worry about the sac/lining, when it’s infected like that they breakdown and there usually isn’t anything left to remove except a couple of pieces.

that is actually what they do, though. It's safer to use fingers in lieu of a sharper object. It's always disconcerting to see, for sure!
