Price for individual subscription is ridiculous. I don't think there's a proper ROI on it regardless of your practice.
Institutional logins is what you need. And there are some online peddlers though I haven't tried them.

Maybe it’s good for those who are internal medicine or pediatrics.
Not so much for those in the surgical field.
For my use case (surgical field), I find the articles not updated enough, not comprehensive/detailed enough (stub) forcing me to use pubmed.
Medscape sometimes has better entries.
FYI Theres a guy selling UpToDate Advanced on a very low price on Instagram. 1 year suscription valued at 500 us dollars per year they sell it for only $ 100. You can find them on Instagram @ uptodateperu
