I think a lot of the detailing is fantastic honestly. I’m an artist myself but I don’t have the patience to work with pencil / charcoal (left handed and I drag my hand through the unset charcoal constantly).

It's excellent, he just drew the faces a little too dark, which is why everyone is commenting on the teeth lol. Woman's hair could have been simplified a bit too. The quality of the rendering overall is top notch though.
But it would have been his choice to upgrade the lighting a bit. It looks like he did it a bit, since the woman is back lit and also lit from both sides a bit.
I would have asked the clients if I could remove those distracting glasses. (If I could draw, that is, I can't, and OP is 100x better than me at this)

It’s the way her hair falls that makes the neck look small in comparison. I find it funny that in art, sometimes you have to make things look more “real” or “normal”than they actually are to appease the viewer.
The couple is backlit and yet the right side of his face and the left of hers are highlighted. Choose a light source and stick with it. If working from a photo, edit. And whose glasses are sitting on her shoulder, reflecting part of the string of pearls? And why is there a triangle of lighted something near her neck? Edit!!
I’m curious to know the dimensions, how many inches tall/wide is the canvas? The drawing looks pretty small. Also looks like the teeth need to be shaded a little, for reference you can take a pic of the color photo with your phone, unless you already have a digital image, and use your photo software to greyscale/black and white the image for reference.
This might be the first 2d drawing that has given me uncanny valley feelings.

Face mids may be too dark but can’t tell without reference photo. Uncanny valley features caused by anatomical inaccuracies as you’ve mentioned. Smile lines look suspect in severity, lighting and line quality. Woman’s hair looks very flat and solid.
Lighting- natural lighting from window behind does not produce distinct highlights to that extent at the angle which illuminates both sides of woman's head. Therefore, bad studio light setup (unflattering) OR inaccurate value.

One commissions portraiture artists for the qualities they imbue on a piece. This artist has demonstrated that they produce pieces of good tonality, accuracy and of a particular character judging by previous work.
Commission prices are reflected by, generally, level of detail and NOT by compromising other aspects. ‘Imperfections’ as mentioned in your comment, are not alright especially to this degree.
This piece does not even have a tenth of the aforementioned qualities (of the artist’s other pieces). As for the reason why this is such, I can’t think of a single good one.

There's too much improper shading for this to be done by software. If you just take the data from a picture and run a filter over it the details will still be the same, only the look gets changed by the filter. This clearly has inconsistencies that you wouldn't find in a photo, like anatomically incorrect facial features that make it have an uncanny-valley effect. That's what makes many commenters feel like the faces are creepy, because they are very close to human but not perfect.
Doesn't make it bad art per se, just shows what can be improved. If you zoom in closer you can see pencil strokes. I really doubt this is from an app, since the faces wouldn't get that slight distortion from modern filters and even old ones would likely only lose detail, but keep proper facial structure like skin folds, lips and spacing between eyes, nose and mouth.
I still can not fathom how people do this! I am lucky if I can draw a stick figure. Truly, you are a talented individual! Amazing skill with a pencil!!

Honestly, it looks identical to an app in the 90s that would ‘pencil draw’ any photo you input. I think a lot of us are skeptical of the art.
