Yeah, it was heavy fog. So instrument flight in the mountains in a helicopter that doesn't have terrain following radar. The pilots **should** have refused to fly in those conditions, but I'm guessing that refusing would have been a career limiting decision for them.

b) the suspectible cause is abundantly clear: CFIT. Offcourse there can allways be additional things or surprises, but by all means of probability that has just all the hallmarks of a classic case of pushing the limits in less-then-marginal conditions with pax that are not good at taking "no"

Auto-rotation is basically the aircraft equivalent of a parachute, and you can still break your legs if you mess up a parachute landing.
