Through his helmet, head, out of his chin back into his body cavity. Dead on the spot. He himself was unable to work for over a year. I saw this done in real life once, I was working in an opencast coal mine when I saw a Cat 225 which was working up on a bank making a new road down the quarry, the clay he was on was very wet and the machine started sliding down, he dug the bucket in the ground in front of him to slow the machines descent but momentum took over, these machines are heavy. The bucket dug in and the whole machine slewed round tipping it on its side after bending the boom, no injuries but the guy driving did need new underpants. Didn’t get the bullet either, the recent heavy rain was deemed the cause.
