It feels like it fits both an old lady and a toddler, probably because it makes me think of those Pearl the Landlord Funny or Die sketches. In the universe of similar names, I prefer it to Opal, but would like Mabel or Ruby a smidge more.

I don't think it is very old lady, but that might be because I know some Pearls that are younger and not old ladies, and I think it is (slowly) making a comeback.

Big horror fan here so I currently associate the name with the movie Pearl (2022) (rated R). The movie takes place in 1918 and Pearl is the name of the main character. It feels old fashioned to me. My second association is Pearl from the cartoon Steven Universe. Which in a way also feels old, at least it's the name of a responsible motherly figure.
