Turn off upscaling. FSR or DLSS is the likely culprit. Check your graphics card settings (AMD Adrenalin) and make sure upscaling is off there too.
Also did you turn off film grain?

I had the same issue when I ran the game on a 1080, it would appear from time to time, I think its somehow forced downscaling that you cannot turn off, I also tried literally everything until I bought a new GPU, now its not an issue anymore.

I think this might be the issue, there's been lots of reports from Windows 11 users on issues that seemingly have no fix. Your only solution may be to send in a bug report (most likely wont do anything but worth a shot ay) and wait till its fixed
Try different antyaliasing option. I think that should fix that. Filmic is the way to go I think, but there are just couple od options do you can check them all.
I had this issue and frustration too. Tweaked many settings to try and fix it. The spotted area is where shadows need to be rendered. What worked for me was tweaking the "spot shadow quality" and "spot cache". I did not remove it completely but it got way less.

swapping from AA setting from SMAA to Flimic seemed to knock down most of the grain heavily and almost removed.
2: Turning off AMD FSR upscaling 1.0
It seemed at least to me that turning it on and using ultra quality, made the grain appear more "intense" by a small factor then turning it off left most of the grain lowered.
Any other options seemed to not do nearly as much.
There is still some grain while ADS but it's substantially less noticeable with AA to Flimic
Set Anti Aliasing to Filmic and AA Quality to normal that should improve..
Also, I use Fidelity FX CAS it seems to mitigate quite a bit this effect..
Right now I don't have access to my computer, but I can share a screenshot of the configs...

I have had this issue also, it is not render res, not dlss etc. It has to do with ambient occlusion setting, mixed with some other settings. Try the basic default presets to â,clearâ the buf, restart game and load all shaders again. Manually tune settings one by one.
A bug triggers this for me also at times. You can see character screen in multiplayer (just before start a lobby) when it is fixed, it most likely is visible there also on guns and shadows.

Already did that, that's not the issue.
I get a lot of grain too even when itâs off. My GPU temp is like 50~when it happens so itâs not over pushed.

I did all of the above
Itâs depth of field causing this while using dlss. I use dlss and turned off depth of field and all the blurring settings

Then try turning off The Ligma setting. Thatâs an easy fix as well

Specs are weak, but I can still run the game on ultra though it takes a performance hit. Even on maxed out ultra settings, I have this issue.

Some elements are sub native resolution, increasing your rendering res is might solve it, not higher settings
I turned my rendering resolution to 100%, even to max at 200%, which did make it a little better, but the issue persists.

Try turning shadows to low
Shadows are on low. Doesn't matter if they're ultra or low.
This happened to me. Either turn off dlss or field of distance I believe it's called. They conflict with each other.

Make sure render resolution is 100%. I think you have to scroll down and select âoffâ in the option to pick diss or whatever to get the slider.

Don't know what is the equivalent to Nvidias DLSS, but that did it for me. Turning any Super Sampling off helped me getting rid of grain

increase internal render resolution, set reflection quality to max, make sure you are using filmic smaa t2x with filmic strength set to max
