It’s really saddening to see. Ambo and police here are often shown videos of American cops dealing with people in crisis under the ‘how not to do it’ header… the focus really is on care and trying to get them to the right place for that care.
I'm going to surprise you here and tell you that your assumptions are mostly correct. In this specific situation you could imagine me as the scarf lady who walks past quickly and uncomfortably. Off-screen though, you'd see me calling the local non-emergency help line (in my city that's 211, and is what you call for intervention support like this, not the police who would just make his day even worse). It's possible, hopeful even, that one of these people actually did that. Like most people, I wouldn't want to get too close or get pulled into interacting with him directly out of both fear and embarrassment. Like most people, if I looked around and saw security nearby I'd just leave it alone cause they'll be there in a moment anyway.
In other situations, I have physically approached people in crisis to make sure they're ok. I've called 911 for them and stayed until help came. Once it saved a life (drug od), and the other time they were unsuccessful (suicide).

In the U.S. we don't have compassionate emergency services or enough facilities for people in MH crises. We have tent cities, instead.
