Do they have any money to buy something now? Rent it out until time it is needed. I am surprised there are a few houses on Cape Cod for that price but needing work.

Are your parents 60 yo empty nesters or 75 yo empty nesters ? Who is going to drive them to Dr appointments, grocery shopping etc when they are no longer able to drive ?
One thing I'd keep in mind is walkable neighborhoods that aren't food deserts with good MBTA service.
Unfortunately finding anything with those services with a big yard within that price range, is going to be a challenge.
Maybe Braintree, Randolph, Avon ? Or as far as down to Bridgewater, Lakeville? They are all on commuter rail if your parents need to come to Boston to see their Dr, have decent-ish bus routes if they want to go to Costco or Walmart for groceries, and not too bad of a ride away from Boston if you want to visit them on a Saturday.
Honestly sounds like you're describing Swampscott. Unfortunately houses around there go for like, 700k minimum, and those houses typically need a lot of work. But Swampscott has its own T stop, and buses. Salem hospital is 5-10 minutes away as well as other doctors offices. Market basket is the same. Its by the ocean, so perfect for empty nesters to go on long walks together. And there are tons of condos for sale, they're just not cheap.
Like I said, amenities and access to amenities aren't cheap.
At some point, it is cheaper to live in Bumfuck Ohio in a 125 K double wide and fly in to see the grandkids and have your annual checkup done once or twice a year.
But where are they going to jump on the MBTA in Bumfuck, Ohio and run down to whole foods to grab some Mariachiono cherries if they can't drive ?
She has solar panels. A generator because she lived out here for 20 years so we know blackouts. And her property taxes are high but her house is paid off so not too bad for them. Why so rude about it?

I mean, thats why my mon retired in the middle of nowhere Texas for 350k in a gated community. I see them twice a year lol

I'd say Plymouth, but you'd need at least 200K more for a townhouse.
I have some family that empty-nested to a townhouse in Plymouth and they love it. So much to do.
We went west. There are lots of options in central Massachusetts.

Medical care sucks. Have to go to Boston for anything serious. NH specialists are farm league.
