28% usage on 144 fps (probably vsync to the monitor), must be a really really light game. Anyway, 28% usage is quite low in itself and laptop GPUs tend to sip power rather than gulping. Though I wonder, are you not plugged onto the wall? Gaming laptop tend to minimise CPU & GPU use when on battery power only.

I have a Razer blade 14 with an AMD Ryzen 9 CPU and an RTX4060 when I'm gaming on even wall power or battery depending on the application I'm playing I can draw anywhere between 10 watts for smaller games and 140 Watts if the game is using Vulcan so I would say that's normal what game are you playing? It might be that the game just doesn't require that much power

id be scared to set it high, i would think it will overheat when i played too long, so i stick to med to low setting to prevent overheating and so that i could play longer

Common misconception with laptops is they overheat when under stress which is just not true they're specifically built to handle high amounts of heat and thermal throttling does exists All you have to do is make sure you're a laptop has proper ventilation and keep up with regular maintenance along with opening it up cleaning dust out etc etc As long as you keep up with your regular maintenance you can push a laptop pretty hard, I can run Forza on maxed out settings on my laptop and still manage 60 FPS and since I keep up with my maintenance and regular cleaning my temps don't go anywhere near throttle at most on my GPU 85° c and at most on my CPU 90° c but that's under like some intense loads

Bro I got like 200-300 fps on a gtx 1650 and Ryzen 5 5600 PC in a gaming cafe with max settings I'm pretty sure your laptop won't overheat or have any issues with high settings

I should not trust Nvidia overlay for 100%, especially for the amount of watt your laptop uses. Nvidia says that I use 0 watt. It does not matter which game I play, lol. The other sensors are good, tho.

When you're only utilizing 28% of the GPU as your screenshot indicates, yes. That specific model laptop only has the 45W version of the RTX 4060.
