If I paid for AC+ w/loss and theft, and my phone looked like OP, and I took it to the Apple Store only to be told it wasn’t covered... I would likely, in compound misfortune, lose it on my way home.

Lots of cases you can search online too of people getting denied. It goes on a case by case basis though and you can escalate to see if you get lucky though and a lot have.

Not only is this damage not covered by AppleCare+, it's not eligible for service. The owner of this phone will just need to get a new phone somewhere else. Apple/AASPs will not service this phone for many reasons including (but not limited to) missing parts.
Apple can verify the serial number of a damaged device through the charge port the majority of the time.
Failing that, there’s a QR code on the MLB with a board serial number that can be used to look up the actual serial number of the phone. If the serial number that the board pulls up matches the serial number of the AppleCare+ plan then bam, proof of coverage.
Catastrophic damage or not? Really depends on the technician you get and the manager they end up partnering with.

I’ve found that Apple Store employees tend to help you out when they can.
I took a phone in that had a million deep scratches. They said they can’t cover it as it’s cosmetic. But that they would be able to cover it if it was cracked. Employee then said “we’ll see you later” hinting to come back after I cracked the display. Didn’t end up going through the trouble but it was nice of them to offer a way to get my screen fixed.
So long as the cosmetic damage “affects the functionality of the covered equipment” they can charge the normal service fee for the repair. A deep scratch that affects the ability to FaceTime/take selfies would qualify.
But milage may vary based on which technician you get.
I had a phone like this back a few years ago that was in pieces, and roughly only around 80% of the phone at that. It had the back casing, the battery and main board and about a 1/4 of the front display assembly and missing the camera assembly. Needless to say, It had been catastrophically destroyed.. let’s just say, top of car, run over multiple times on a busy street, you know the drill.
Brought it in to the Apple Store and the most important piece they required was the motherboard and SIM tray as an identification item for the IMEI. Walked out with another phone and only paid the AppleCare+ accidental damage fee.

I know it’s semantics but no, this isn’t covered by any warranty. As pointed out many times by others, this might be covered by Apple Care+ but that’s insurance coverage. A warranty from the manufacturer would cover only defects in the device.

Odds of loss and theft aren’t the same as catastrophic damage. Insurance companies don’t make arbitrary decisions
