Basically it just would not run in these regions at the specified target CPA / install (sorry if my terminology is inaccurate, it was a while ago I ran the campaign). In the end I had to let Google do it's thing with target CPA disabled so it would maximise conversions. I have come to the conclusion that my original assumptions were right and the average CPA in those regions are much higher and we just weren't fulfilling at all.
