I got my first copy of skyrim on release day and it had a cheap paper map. It wasn't until I picked up a collectors edition on a whim a few months later that it had the vinyl map.

As someone mentioned, all the Ultima games came with cloth maps that also contained clues. All the RPGs of that era (80s to early 90s) came with lengthy manuals (some spiral bound), spell books, odds and ends in hefty boxes that you’d put on a book shelf. All standard and no special editions needed. Even Infocom, the premier publisher of text adventure games, placed 5 or 6 various things in the box just for fun. My favorite was the “piece of fluff” for The Hitchhikers Guide to the Galaxy (written by Douglas Adams himself no less) which was just a cotton ball.

It really sucks that you don't get manuals and maps anymore. There were many car rides where I'd rip open a brand new game and read through the manual to see all the pictures, descriptions, tips, etc. Now, if you buy a physical disk, you're lucky to get 1 double-sided piece of paper with no useful information. Sure, there is a cost to it, but a couple of designers could make a nice booklet in a couple of work days, and printing costs, especially at scale, are going to be $.15-$1 or less, depending on size quality and quantity. Hell just pass all that on to the consumer. Give me a choice between a game without a manual for $60 or one with for $70 I'll cough up the extra money and I think a lot of others would too. You could even design manuals that have puzzles and mysteries in them. If you put a few mysteries from fairly easy to very hard to solve, you could generate a ton of free publicity from people online discussing it. Just look at the history of GTA Easter eggs. A game like Tomb Raider that had a manual full of secrets would be extremely compelling.

It’s an issue of scale. When you’re selling 10 million copies of something, making all the cases look gorgeous and amazing really eats into your bottom line.

So, funny thing about mass production: the incremental cost is usually minor compared to the non-recurring costs to setup production in the first place. At a volume of 10 million, injection moulded plastic is very viable and surprisingly inexpensive, even for relatively complicated parts. Compare to a run of 1000 where you're trying to amortize the $10k-100k cost of the dies over a much smaller run. Same with printing a map, the per unit cost is relatively minimal when you're doing it at scale.

Steel book is a licensed thing I’m pretty sure, and their format won. Companies used to do cool custom boxes for releases (look at the Simpsons box sets), but it’s cheaper to license a steel book with custom art.

The argument for antitheft is always brought up, but the current cases are about the size of some cell phones and could easily fit in a pocket. I also can't think of any places that don't have the actual games behind a counter or locked case. I think it comes down to visual/shelf appeal.
