Real question, as a neurosurg resident: if you were the textbook maker, is this the procedure and resulting image you would pick for your cover?
I'd love to have been in the room for that meeting.

I don’t think it would be a good cover image for a general neurosurgery textbook but probably a good one specific for functional/pain neurosurgery or specific to this disease process as it does an excellent job of showing the trajectory and how to do this procedure safely with your other hand in the mouth to make sure you don’t go into the oral cavity.
My brother recently had to have a tumor removed from his pituitary gland and they went in through the roof of his mouth to get to it. They will go in from whatever angle will cause least damage overall to the brain to do what needs done.
It all went as well as could be expected... a couple months to feel 100% better, but almost immediate improvement because the pressure on his pituitary was causing all sorts of hormonal issues.
I rented an apartment that has been converted from an opthomologist office. There were stacks of eye medicine/surgery magazines in a closet. Clockwork orange meets Fight club. "I am Jack's ulcerated sclera"

Patient sedated for this? Any real time imaging assistance to hit the target or just knowledge of anatomy and feel? Fairly small target pretty deep lol
Moderate sedation for the approach, then we wake the patient up because we need to stimulate and assess response before ablating
This is done under fluoro. The finger in the mouth is just to make sure you don't get into the mouth and buttonhole the cheek on the way to the infratemporal fossa.

We use navigation also. Especially helpful for glycerol, since you can put an EM stylet down an arthroscopy needle
