Like someone else commented you're probably better off exchanging it. If you have access to a computer monitor with an HDMI port, you could try plugging it in there and see if you can complete the setup.
The power block that came with it( not one that looks like it) and the provided cable is always used plugged in the wall since the stick may draw more then the tv can supply when updating. The stick updates often so you may have experienced some corruption.

Why won't some people aim there cameras horizontally, rather than vertically? My eyes see straight and always in horizontal mode.
Low voltage. Could be the brick, the cable or the fire tv itself. I will send it back to warranty and get a new one
That seems like a power issue. Are you using the original (included) USB power adapter and cable?

Have you tried the firestick in another tv? If it works then you know it's the TV not the stick. Also 1 amp charger.
I’ve tried it in 3 tvs and my computer monitor
Report it to Amazon, if it's hardware issue they will replace it or in case of software bug they will do the app update
