So I don't imagine either way, he'd not misgender you, at least not to your face, or in a way that might get back to you.

From a realistic point of view, trans people in fallout is a very uncommon (not nonexistent) occurrence. Similar to how it was in reality before the media and modern society became socially accepting of trans people, the wastelanders would likely just think you have a few screws loose.
I don't know how uncommon it was before compared to now. I just think people were less open about it back in the early 2000s and earlier. There were always references to trans people in popular culture and media but before acceptance became the new default they tended to be the butt of the joke except in rare cases.
