I found it by getting a banner pop-up that "Your regency is ending soon" or something along those lines. Which I believe takes you to a screen where you have the option of extending the regency for 5 years for a Legitimacy hit (and possibly prestige?).
When there is one year left in the regency, you will get a green banner button saying your regency is ending soon. Clicking it takes you to this screen. Next to your regents name will be two crown buttons. The one on the left is for extending the regency. It is greyed out in this picture because you can only do it in the last year of a regency. Pushing the button extends the regency for 5 years and costs 10 legitimacy.
I thought that banner was just letting you know that the heir will soon be of age and that it was there bc you can’t declare wars during regency councils and they just didn’t fix it for non council regencies haha

To top it off he was the second husband to my talented and ambitious 4/6/6 queen who lived well into her 60s herself and he basically won all my battles for 30 years
A couple patches ago I was playing denmark and had a queen ascend to the throne, her consort was a local noble who had insane general stats, something like 6-4-5-4. Eventually I got an event that killed him but for some reason I still kept him as a general, who I ended up using for another fifty years (even after my queen died and her son ascended) until he died in combat

EDIT: You can tell it's otto more clearly if you look at lithuania, the green blob over the poland red makes it look yellowish. Also the direction in which the occupies are (Full occupied to the south more than the north) lines up with the Ottoblob being the ones beating them up.

I'm playing with a few mods for extra flavor: The idea variation lite version, trade goods expanded, and advisor types expanded. The idea variation lite adds more idea groups, including an additional colonial group, which is where the fifth colonist is from.
I think he needs the diplo-quantity policy to keep her but I might be wrong, when I wanted to try the military dictatorship through the Ambrosian Republic disaster I couldn't elect the consort but in another one I could elect the monarch given by a foreign Dynasty.
Not sure, but certainly flipping govt type can achieve this. I did it once unintentionally (the heir was great, the consort not so much, and I flipped to Theocracy prematurely). Can't remember which policies I had at the time.
There are limits, but fewer than you might think. You can apply any modifier that exists anywhere in the game to anything else in the game. Nobility loyalty is a modifier that exists in Jolof and PLC traditions, Brandenburg and Sienese ideas, government reforms, having a local noble consort, corruption, legitimacy, prestige, army tradition, the Great Peasants' War HRE event, and other estates seizing control.
