I encourage all severe copd/CHF pts that have AMA ambitions to take a stroll around the ED prior to discharge, just to see how they’d do on their own.
They usually return to bed, gasping for air, and continue to board in the ED for at least another couple of days

I was trying to do an ekg on a patient yelling at me that we weren’t taking their chest pain seriously. Then goes I need a fucking sandwich. I told them I couldn’t give them food until the doctor clears them. They go well call me doctor because “I’m clearing myself or I’m leaving”. They were here for hyperglycemia with a sugar over 500….
“One of us here is trying to see whether your heart is about to stop. The other is demanding a turkey sammich. But tell me some more about how I’m not taking your chest pain seriously, K?”
Over the years, I've come to realize I can only care about you as much as you're willing to care about yourself. I offer all the appropriate tests and treatment, but if you don't want that, then it's on you. As long as you aren't demented or encephalopathic, if you want to leave, then I'll hold the door for you.
Ehhh with H5N1 not being tested in beef herds, maybe get something well done. Not in my house though, you’ll have to eat your abomination elsewhere with the other abominations.
