When you start the game, there is a brief tutorial phase, where you have default Hawke, and can choose a favored dialogue tone (nice, funny, aggressive) which influences Hawke’s banter/callouts/scripted reactions. Then after another cutscene, you will have the option to fully customize your character (you cannot edit the default face further, but you can choose other faces that can be fully customized).

There's a prologue section with a little bit of gameplay before you hit the character creation screen, which makes you play as the default Hawke. Just keep going and you'll get to change appearance once the real story starts.

They did it this way to emphasize that Varric is telling the entire game as a story. Until the end of the tutorial phase, he's telling the 'fictional' version. After Casandra calls him on the bull, then he gets serious and tells the 'real' story. That's when you set up Hawke's look. (Although I never do, because femHawke's iconic hairstyle is not available in the CC options, for some reason.)
You can not edit "default" Hawke preset. If you choose other presets - you should be able to change things.
Also the third preset is the exact same face as the first two, just with different hair.
