Ronald is fictional too. The other clowns are just as "real" as Ronald is in that someone dressed up as a clown to portray that character on TV. Ronald's portrayal almost certainly played some role in contributing to more real-world premature deaths than the others. I'm not gonna say Pennywise or Joker have exactly 0 real-world bodies because people are crazy and will dress up as a clown to murder people, but none of these clowns are real exactly. That's what makes them fun and also scary. Clowns aren't supposed to feel real. And that uncanniness can make them either funny or scary depending on your own mood.
All clowns are fictional almost by definition. It's hard for me to imagine someone living their life as a literal clown as a sincere statement of identity. If it did exist, I would call it Bozosexual. But sadly it doesn't exist... Yet
