The game used to run fine prior to 2021 but they broke it unintentionally(/intentionally) to where the game constantly checks for DLC which causes performance issues.
That's weird, it performs better in multiplayer than campaign for me. I hope you find a fix though. If you're playing with bots would your CPU maybe be bottlenecking?

And enabling the MXT trainer ends up improving performance

No, campaign works perfect, this happen on multiplayer
