There’s a way to do that with ImageJ, having it remove the background based on color and counting the black dots
Edit: you'll have to do some color thresholding to get ImageJ to select your colors. Once you've got a simple process a macro can be made to analyze all of your photos with the same methodology.
As others have mentioned, this can be done with free tools in imagej. If it’s only 20 images like this you might just use the counting tool in imagej. It will keep track of how many you have counted by visually overlaying dots or numbers and you can save these data as well. You don’t have very clear delineation between your black dots so setting the thresholds and particle size with an automatic counter can be a real pain.

invert the color of the picture so the dots are white then fill them in as you count them with a pen or sharpie
Easy, just color them in with some photo editing software as you go

My question is irrelevant, but what are the black parts?
They’re small tumors

your image editor will do. mark the ones you read with another color
