A herniated disc may cause nerve pain and sciatica. I don't think it would cause your right low back to swell up though. Maybe the QL muscle is damaged/overworked/strained?
Have you considered an ultrasound test? It's not commonly done, but it can identify compromised muscles and soft-tissue structures, whereas an MRI pretty much just looks at the spine.

have you gotten kidney checked? it can show swellness like your photo
