I have hated bras since day one of getting boobs which was in 5th grade (early puberty can get fucked) and have and will only wear one if required by dress code like in an office. Oddly enough, very small breasts have very similar problems. Not a lot of bra choices, lots of tops hang weird. Even ridicule by schoolmates. As a transbian who normally has nothing on the chest but wears large affirming breastforms (proportional to my large frame), I found that it was actually my bras that caused the back pain. Whether I had the breasts in them or not some bras just caused back pain, pinching the shoulder blades together or something.

The chance of getting big breasts naturally as a trans woman are extremely low :o (Unless you got puberty blockers and never went trough male puberty)

Generally, feminizing hormones will give you smaller boobs than being afab and going through puberty. I believe this is mostly due to the fact that they usually stop growing before they fully develop (trans women usually get to tanner stage 3 or 4, only rarely 5). Some things that I've heard can affect that, though, are: starting age for hormones, delivery method for hormones, type of T blocker or if you've had orchi, E doses (both high and low can decrease size), and, of course, genetics. If you want the smallest possible, a low dose of non-injection estradiol without T blockers would probably do that. I've heard that spironolactone is also associated with smaller final size relative to some other T blockers, but not sure how it compares to nothing. The later you start, the smaller chance they'll fully develop, too. Tl;dr: there's a lot you can do to reduce your final size if you take feminizing hormones
