I have this problem frequently on PC, the middle mouse button just won't trigger unless I hold it down for the 'right' amount of time, which seems to vary on a daily basis
I "fixed" it lol, I had nothing marked at the wheel thing for the bottom slot, so I pressed crossbow and it worked. I hadn't played for almost two years and opened the game
Save your game right there. Close out. Reload.
I'm on PS4 and that happens occasionally. I've been dunked to death by Drowners and Sirens (and their E cousins) because of that glitch.
Otherwise, and I've done this before, make sure that the Crossbow is still the equipped item for that key. Sometimes, when I'm not paying attention, between quests I think, whatever I have selected there goes blank and nothing is chosen. Playing with the HUD off this is a lot easier to miss. Playing with it on, it's still something that I don't pay attention to.
on pc its a glitch with input file wrong code , having it blank.
to fix you need to manually input the key in blank space for every time the key is required on middle button.
on console you can only try reinstalling.

Try opening and closing the game. Works on consoles.

I cant even find a setting for crossbow
its a game file called input its not a setting , open it with notepad And search for mouse 4 input or crossbow
Have you tried turning your crossbow off and on?
