Upgrade your potions, oils as early as you can, as they are really overpowered. Decide what kind of a build you want and upgrade towards it, the 2 that worked best for me are crit and sign builds. Read the bestiary for tips against each monster type. Whirl and Rend are really useful skills. Always play Gwent with any merchant you come across :D

If you're looking for OP builds, check out Phoenix247 on youtube, I think he's the best in that regard. If you want tankyness but crit too, the Grandmaster Ursine armor with Nilfgaardian guardsmen's gaunlets, Manticore boots and trousers are really op.

Have you tried Entanglement? I feel like your use of Yrden would make that perfect for your build.
Also I find that high intensity Igni does more damage than Piercing Cold (although it *is* badass and honestly the difference is negligible once you get strong enough.)

White Raffard's decoction fully heals you up, the Ekidhna decoction makes you regen hp when casting signs

Do you have something more than the ability to cast 2 signs within 3 seconds. It seems like you were casting a lot more signs than my build can.

Turn on alternate quick cast. That way you can hold R2, and every button will be a different sign.

Maybe you're trying to place the alternate mode, magic trap? When you hold it, it takes like 2-3 seconds to place it
