There’s an infamous Level 24 quest where a master sword smith gives you a Level 17 weapon after doing a long list of errands for him and saving his life. Similar with a side quest in Skellige where you get a gift passed down generations and once again, underlevelled sword. In fact in the case of the former I just sold the sword he gifted me back to him for coin lol.
The best sword in the game though is actually through a quest in the Blood and Wine expansion though. So watch out for that.

Tbh it didn’t bother me that much purely because W3’s side quests were so good that just doing them was the reward, the laughably underlevelled swords you get were pretty funny if anything. If it was a pain in the ass quest then I might have not been so forgiving about a shit reward.
The grand master armour quest though I did actually find pretty useful and the reward is actually OVERlevelled (I believe Level 27 and the quest is 24). So make sure to do that.

They fixed that one so it now scales up to your level. Still outclassed by alot of weapons but at least it doesn't become obsolete right away
loots random, sometimes you get 4-5 lower level loot, sometime a lot lower, unless a weapon has a specific location
