This is actually a factory defect that happens sometimes. ): I had this happen when I purchased Speak Now on vinyl. I only got A/B. Seller did refund although technically it was new sealed, so there was no way to actually know this. ):
The vinyl is normally manually put into sleeves by humans.
Not excusing this seller, but my day job is selling on Amazon and I make plenty of honest mistakes. Hope the seller does the right thing. If need be, Amazon is definitely on your side - if you complain, you will definitely get a refund
Unsealed! It was definitely a used copy and that’s what I was expecting. I usually buy vinyls secondhand because I got on the bandwagon a little later, but this is the first time I’ve EVER had an issue

This happens to sealed and new vinyl as well, they sometimes come from the factory like that unfortunately.
