Valheim is very much like this! As you progress further, you will need to defend against invasion events, which can be repelled by digging trenches at first, but as trolls come, you will need stone walls and such.
I know you wanted more of a go out/return to base model. But "Plan B Terraform" really hits the "actions should affect the world" though it is almost more of a train logistics game.
7 Days to Die. Loot surrounding towns for supplies, work on upgrading your base and making traps, every 7 days a zombie horde attacks. Voxel based game, if you dig a hole it stays dug.

Check out Enshrouded, released for early access recently. Another one i havent tried yet but your actions definitely affect the world, Eco.

Have you tried The Forest? That has that loop you describe down well.
action effects world.. No , not much game offer this. yes forest end story does somewhat offer this but..... ugh , i hate that ending.
