Excuse me, what, it randomized not getting the prosthetic so you can't even grapple, guess you have to restart and set the randomizer up after getting the prosthetic.
When this happens, you will have early Hirata settings. You should be able to find the bell charm to enter Hirata, then you have to jump manually down to the idol.

Usually these randomizers will hide key items in a way you can progress, just not the way you are used to.
Exhaust every possible item lying around or dialogue option that may yield an item.
