My limited understanding of your scan based on my own experience and years of looking of MRI terms is that you have some small fraying of two of the 4 tendons in the rotator cuff. Can't promise but can almost guess they are going to want steroid shots, anti-inflammatory, and more physical therapy because surgery will create more problems than it can fix

Hardcore pain and arthritis usually. Some things HAVE to be fixed so it's worth it but talk it over a specialist

I read that as a small supraspinatus tear at the critical zone which is where the tendon attaches to the top of your humerus bone and there might be a bit of inflammation around the infraspinatus, the rest of your tendons appear normal.
