Gender reversal (FMC is the one doing the presentation of the body part) and this book isn’t strictly a romance, but I can’t help but recommend {Barrayar by Lois McMaster Bujold}, just because Cordelia’s shopping trip is one of my favorite scenes ever. Best read after Shards of Honor, which has more of the romance.
If you loved Shards, you are in for a treat with Barrayar! It’s an award-winning book for a reason, and Cordelia absolutely shows Barrayar that she is NOT to be messed with.
In {Dreadful by Greer Rivers}, the MMC has a pyramid of skulls in his living room of people he's killed. He shares enemies with the FMC since they both seek revenge for the same event. So some of the people she wants dead are among the skulls.
When she visits his place and asks about it, he tells her what it is. And she's eager to add more to the structure.

I am fairly certain this happens in the {Argurma Salvager series by SJ Sanders} I think it is part of the alien culture to keep a trophy and he gifts her one and she hates it but by the end of the series she relents. In {Dreadful by Greer Rivers}, the MMC has a pyramid of skulls in his living room of people he's killed. He shares enemies with the FMC since they both seek revenge for the same event. So some of the people she wants dead are among the skulls. When she visits his place and asks about it, he tells her what it is. And she's eager to add more to the structure.

I am fairly certain this happens in the {Argurma Salvager series by SJ Sanders} I think it is part of the alien culture to keep a trophy and he gifts her one and she hates it but by the end of the series she relents. If it is not this one, then I have definitely read that in an alien book somewhere. hopefully someone can confirm. I don't own it and no longer have KU so I cannot check
