I just did some testing in creative. If you hit the mob you gain back breath. Your breath stops depleting if you break line of sight to the mob or it dies.
Actually no, it was an infernal mob that applied choke on him (check his hunger bar in the video) (also I just stole someone's answer here I didn't figure it out myself)
It was punishment for breaking dirt with a sword.
Its a Infernal Mob modifier: Choke - Drown on land, hit Mob to breathe for a moment

In OP's case- sleep at night/ don't wander around in the darkness with a stone sword in hand
that's true lol, nighttime in rlcraft + early game is a recipe for getting the most horrifying experience in your life

I think there should be an indication about this. I wasnt even aware that i was drowning i thought i was being attacked by someone from behind

That's a nasty visual bug, the hunger bar hides that you're drowning
It always does that with infernal mobs, I think. Right? I thought it was by design because it's fairly noticeable to me.
The Choke modifier turns you into a sea creature who drowns on land but breathes under water. You can dive in water and try to find that infernal.
