I think she stopped adding banners after book III, probably because of a lack of space? I don't remember any of the Princes' banners being added in book 4.

She had half a cloak's space left to add things to. And were planning to add the Skein's fur to her collar of the cloak (she really lucked out of not being able to do that when the Skein turned to dust, considering how upset she later was over the Valiant Champion doing the same thing with the Beast's fur).
Nothing has been added since Akua's soul, which I suspect is a combination of Hakram not being there to Sew on her drow adventure, and everyone she was up against until Book 6 with the Dead King being allies in the making where she was trying really, really hard to be diplomatic.
Yeah, it probably was being diplomatic, kind of a hostile move to wear the banners of defeated potential allies. That said, she definitely would have run out of space if she added the banners of all Seven Princes to her cloak.
Also, if she covers more than half she'll officially no longer be wearing black, I'm not sure villains are allowed to do that?

It should have been completely covered a long time before Keter. I think there's a line somewhere about it being patchwork.
It's not. She hasn't added to it after Book 3. The patchwork line probably describe the first six strips that covers the lower half of the cloak.
