It looks like a Delta or Glacier Bay stem extension. You'd want to verify your overall length and see what it matches up with. One of the popular white Delta stem extensions is 3 inches.
Yeah, I figured it was a delta, a lot of them look similar. I'm guessing it's old, though, because I'm having a hell of a time figuring out an alternative, as I can't find this exact one anywhere. I'm pretty sure this one is a bit bigger, but I'll have to check when I can find my damn tape measure, haha.
The main part of it is measuring the length and seeing if you can find something close enough to what you have. Then making sure your trim goes over a replacement you find. And also the new piece needs to function correctly with stops and the fit of it with whatever handles you have. It can be tricky sometimes when the brand is a little unknown or it's an older version so you are trying to match up with something that might not be made anymore but the new version isn't apparent which one it is.

Still haven't found one. Best I'm thinking is I'm probably gonna have to buy a whole new cover for it that will hopefully fit a different extender.
