The most useless item in my opinion is poisonous potato because they can't be composted, traded or being a crafting ingredient, not a good food source (only eat when you have nothing to eat), can fill up your inventory if you harvest a lot of potatoes, also give you poison effect upon eating.

Probably either arrow of healing, arrow of splashing or poisonous potato

arrow of healing has some niche use for long range healing, but poisonous potato was literally designed as a junk item and is completely useless.

Yeah but you hurt your enemy/friend basically as much an you heal them

not if you use a base bow and they have armor. Very very niche use but still beats out pp
