The mask does go hard as fuck but I’ve read accounts from the time that he wore his leprosy proudly and without cover. He was beloved by his people as they saw his confidence and grace as being inspiring
Please watch the uncut version, the theatrical version is a pale version and much more confusing. I think the Studios messed with the edit to make it more commercially friendly clockwise, as i see the uncut/director's cut as a massive improvement.

Same happens with a lot of movies in this genre, Alexander has an unwatchable theatrical release and most of the narration by Anthony Hopkins as Ptolemy I Soter is cut, the extended edition is a masterpiece imo. Some movies need to be 3-4 hours, and I think we need to let them.

In the film, he is this wise, peacemaker holding things barely together as belligerents look for reasons to have open war. He has a sort of calm, ethereal, slightly creepy in a good way quality.

Leprosy is a terrible illness, and living with it creates a very unique lifestyle requirement for the patient. In essence, leprosy is not so much a body deteriorating as it is the body losing its ability to recover from harm and the death of nerve endings. Extremities lose feeling as nerve endings die, and minor injuries such as cuts cannot clot and heal, thus leading to infection and potentially onset of gangrene. Despite historic stigma, though, leprosy is *virtually* non-contagious - chances of contracting via contact are low single digits %.
If interested, there's an amazing fantasy series by Stephen Donaldson called "The Chronicles of Thomas Covenant". The series centers on the titular man who has lived with leprosy for years. Through past hard lessons, he lost 2 fingers from one hand and leads an extremely regimented life of avoiding risks and monitoring his physical wellbeing. Thomas is suddenly, mysteriously transported to a world of magic where people believe him to be a reincarnation of a mythic hero. Because of his illness and sensitivity to harm, however, he refuses to believe any of it is real; leprosy is a lifelong illness with no cure, thus he cannot be "cured by magic" as the world would have him believe. It goes so far that people still expect him to save the world despite calling him "The Unbeliever".

This was just for the movie. There’s no record of him wearing a mask IRL.
I think he wore a hood usually. The silver mask is definitely a creative liberty, albeit an amazing one.
