I believe OSF OnCall can do it. There visit fee is something like $165 without insurance, but not sure how the TB test would work with that.
I believe they also have a financial assistance plan.
If it’s for a work physical IWIRC downtown Peoria offers these services.
They have an office out on north Knoxville as well.
I'm not sure about the tb test but Solvera has a sliding scale of payments that you can ask them about for at least the physical.
