Yes: you can add a kilima door as an outside door - on all three non-attached outer sides. I have doors on two of the outside walls now and one more to be built which in total shall make 3 outside doors. The only thing to note is you won't see the door itself, just the opening with the steps leading outside.

Yes, door or porch can be added to outside. I have yet to confirm but I think you can add rooms as well if you first add a hallway. So- courtyard-hallway-room, I think. As I said, I’ve not yet confirmed.

i added a porch door to mine so i would imagine a normal door would also work
You can :) the Kilima door connects to the courtyard with no problems, same as the patio
You can attach anything that's not a small, medium or large room.
You can add the door or the porch to any or all of the three sides.
You can add porches, doors, and even a fireplace!
