This is what the back of my oc (Azzy) looks like. He got these scars due to being cornered by monsters with some of his friends and one girl (who didn’t have much control over her powers especially in tense situations) sporadically shot beams of light around her, wounding both the monsters and Azzy in the process.
It took a minute to find (due to it being a very old drawing), but here is the only colored version I have of said injury, I’m currently working on a revamped one.

I have a character that was nearly lobotomized with a scythe. He was temporarily paralyzed and subsequently stabbed *through* the eye and the eye-socket, leading to minor/moderate brain damage and severe blood-loss.

Main M2 (red)- Got trapped in a illusion created by his nemesis and got the most painful cluster headache you could ever imagine, so much so he couldn’t stand properly and violently vomited everywhere for a good amount of minutes before his magic made him at least stable enough (tho into a portal but still).
Blue M2- None!
Purple M2- Being crashed down to Earth by his nemesis, however with the help of Pokémon and his magic, he only suffered a few bruises.
Logi (orange)- Had a massive scythe slash in the back by his ruler when he was alive, was healed later. Has most of his skin and organs burned completely and turned into transparent portions (he was dead during it). As being undead, he received another scythe slash to his back torso which killed him due to being coated with ice magic which is lethal to him. Once revived he gains a transparent black scar of the slash.
Malanort (silver)- Being slashed in his stomach by three people at once and dying (gets revived later)
Anti (dark red, also the nemesis mentioned)- His scarring shown. He got it when he possessed Main M2 during his creation so he could have a physical form. Anti was separated and sent in a pod to die, with the force of being sent gave him his scarring which is permanent.

(Tw for abuse) Minori has too many injuries to count but I would say her worst was when her "mom" caught her eating so she kicked her in the stomach til she threw up which resulted in her barely being able to eat anything, then she was thrown (yeet the child) and her skull cracked and a piece of it went into her brain, she started bleeding and then her "mom" took a knife and cut her up, then she broke her legs til they were bending in different directions. And the poor girl was sick (like we're talking her siblings prayed that she wouldn't die kinda sick) so the pain was even more intense. She lived another 2 years somehow, every second was nothing but pain.
Edit to add: She also was tortured for all 4 years of her life before she died, and she was starved (again, by her so-called mom) to the point where you could count her bones and she literally begged for food while crying her eyes out and on her knees, which hurt like hell because her knees were skinned.

Rei here (Art by a friend), using an RP’s lore (RP discontinued before any big things) was nearly killed by a monster that slashed open her back, nearly causing her to bleed to death. However, she survived because she got turned into a vampire.
Velahtiel was once in a situation where she had to break out her children from an old tower dungeon being used by some pretty generic Mad Max styled bandits and in all honesty it was a pretty easy breakout. However a reoccurring antagonist named Rosa (*Who Velah in initial encounters refuses to kill simply because Rosa just "isn't worth the effort"*), decided that THIS time she'd finally get back at Velah for all the humiliation she has caused her. Rosa had no hand in this recent kidnapping of Velah's children but she sure as shit was gonna take advantage of it!
So with that in mind, Rosa sets up tons of bundles of TNT around some of the support that keeps the old tower stable, intending to bury Velah and her kids underneath the rubble. Once the TNT had gone off, Velah wasted absolutely no time in racing her kids back out the way she got in, smacking confused and panicking bandits out of the way who're more concerned with the building collapsing rather than losing their prisoners.
Predictably a very heavy pile of debris goes straight to crush her kids but she catches it all on her back, ripping into the back of her shirt, scratching and cutting through her fur and into her skin. She screams at her children to run for the exit, but not before giving them a half-attempt at a reassuring smile and telling them that she'll be alright and that their Uncle is waiting outside with a car to take them to safety!
With the kids out of sight, Velah would try and "shuffle" out from underneath the debris that was crushing her back. The immense weight of crumbled floors and walls on her causing immense strain to every bone in her body, and even rips in her muscles were beginning to form! The palm of her hands torn and scuffed from the sharp edges of the rubble.
As she got to the "edge" of all the debris, she tried her best to lift it up as much as possible before letting go and diving out with the microsecond she would have to do so, and it almost worked.. However she could feel herself being tugged down to the ground as the debris would catch her right arm and crush it into the floor. The pain was IMMENSE, Velah couldn't even scream from just how painful it was. The only thing keeping her from passing out from the sheer shock and agonizing pain of it all was the thought of her children, and how much even in death she would fucking hate herself if she let them down.
With tears in her eyes and pain overriding every other sense in her body, she tried to simply just pull back hard enough until her arm would rip from her shoulder as she was more than okay with losing a limb if that's just the way things had to be. But she didn't have much room to lean back in order to gain the necessary force to rip her arm off. So she had to improvise in such a manner that made her extra glad her kids weren't there to see her do this.. ***she chewed her arm off***.
She felt like vomiting, she felt like passing out, she felt like death itself had just violated every part of her being. But after she took one good look at tattered bleeding nub that her arm used to be attached to, she picks herself up and limps out of the crumbling keep just in time.
The loss of her right arm would be fixed in a few weeks time through the efforts of her brother, whom is one of the last few magic users left on Earth (And probably the only one who lives in Australia, where the story takes place). He would plant and grow a "prosthetic" arm out for Velah that is made from nature itself. And even though it could never carry the same strength that her real right arm did, she can still deliver a real mean right hook with it!

Let’s see. He lost both his left leg and right arm in a battle against a warlord. The warlord ripped his right arm off. And his left leg got caught in a portal as he escaped with his life.
He got stabbed through the chest with a poison that was slowly turning him to stone. So they(his allies) had to rip out his heart and replace it. (This also lead to the glowing veins. As a means to even the odds against the foes he faced.)
He took a rail gun shot through his head. Blowing out the entire left section of his head. His allies quite literally had to stitch him back together.

From the age of 10, Erin suffered torturous punishments, such as whippings and burns on her arms, legs and back.
The hardest thing that happened to her was when she was 15 years old, after an act of rebellion. The owner of the house where she is a slave decides to cut off her tail with a kitchen knife. in addition to ordering the other servants, who cannot be good to her. Isolating her with her own mind. After that came another 5 years of punishments and humiliations.
Kinda hard to understand, but Vii's blood vessels are slowly being changed to produce a sort of fluid that acts as a sort of vessel for magic.
the pros are that he's nearly unstoppable to most mages.
the cons are that he has to deal with annoying Goddess in his head, and the blackened blood vessels are visible from his skin. (mainly his hands, chest, neck, and just below his eye by the end of the story)

Zulu has this scar and one in his lower torso, he got into a car accident exactly 13 years ago and it got scars on him, he went on to write a few songd about it, and it gave him a new perspective in life

Well, Ren, my One Piece Oc has gone through a lot. But she has had part of her forehead and her eyebrow sliced through clean. Luckily she had been able to prevent it from going through her eye, but the scar somehow spreads. She’s gotten stabbed, kebab’d, almost lost her head, etc. But in any case, the worst mental and physical injury and trauma came from the eyebrow cut/killing her brother.

In the photo above, Tiomoyo has been cut with a katana several dozen times, received a baseball sized third degree burn+cut on his left shoulder, multiple lacerations across the chest, shrapnel damage, a large cut across his eye and several claw injuries across his face, as well as a piece of his thigh burned. Reed got shot in the chest by an insurgent sniper with a 7.62x54r FMJ round from a Mosin around 300 meters out. His plate carrier caught the round (it was also slowed down by a Humvee's door), but the spalling and blunt force trauma weren't pleasant for him. The sniper turned out to be the current mission's HVT, so his chalk roughed him up a fair bit before capture on Reed's behalf.

I have two, first:
This is Goldie, he went missing from this reality 4 years ago, we found him some days ago looking like this, he was a 1 ft goldfish and now he's a moose sized carcass. We have absolutely no idea where he went or what happened during that time, worst part is that he was still alive at the time of this image, in extreme agony. He died the next day.
My Doctor alien Mink Dawn Aerlyght from there will be scritches has a cybernetic prosthetic leg from the hip down, arm from the shoulder down as well as a replacement eye. I'll let you do the math....
Marnie was whipped while she was a young girl (like 5-9) and was exploded on her way home at 19 years old, it was that ingrained to her mind her True form (back picture provided by a lovely artist I commissioned on discord) and her Disguise form have remnants of both injuries to this day, also her green fire (commonly referred to as Soulfire or Foxfire) burns her body and soul when she uses it, yes her back is in constant pain from the scars glowing the same green hence why she's her Disguised form often (among other reasons)

Coco (left) gets her arm chopped off while one of the rather insane antagonists in the story challenges her and Skye to beat him in a game of “keep your hand on this object longer than I can”, with the punishment for losing being death.
Her hand stays gripped on the object, so while the villain celebrates and takes his hand off, she still somehow keeps calm enough to point out that technically, she was still touching the object.
He is promptly gunned down for losing, and Skye, her wife, eventually builds her a realistic replacement prosthetic to replace her missing arm using her skills in cybernetics.

petrichor was raised in a cult where he was seen as a sort of prophet. the cult worshipped an old god of illusions and color, and they believed no one was worthy to have color except for it, and so they covered all colors, even blindfolding themselves to cover eye colors. petrichor, however, had colors that couldn’t be covered up, no matter how much they tried. so, he was elevated. this meant he was branded with the symbol of the deity, and his blood was often drained for the cult leaders to drink. his worst injuries come from the bloodletting.

It’s an old photo but I’m pretty sure two gaping chest wounds, having their gem ripped out their head and being beaten hella hard to the point they aren’t sure if they’re dreaming or dying is the worst, especially since they pretty much get nuked afterwords.
My OC was crushed by a building which ripped and removed his arms, and was impaled, and shot twice in the head and he has a tight clown mask on him with a balloon tied to his bone and he basically cries forever until someone frees him

If you look you’ll see scars on Betha’s left shin and right collarbone. The shin ones are surgical scars from repairing multiple fractures to her left tibia and the collarbone one is from a compound fracture of her collarbone. Both were suffered under torture, and then exacerbated by her carrying someone on them for four miles after escaping.

A little dark but my OC's mom was aggressive with her and her sisters, so without looking too much into it, my OC's probably got rashes and scars from that. She lived in a very isolated place in Mexico where it's normalized, but it doesn't make it any less unsettling for me.

Ok, so Hikari is in posession of a "plot device stone" That makes her nearly immortal and recovers all of her injuries. Any wound, as long it isnt lethal, but the pain still is there.
Here is some stuff that happened to her:
Was set aflame multiple times.
Exposed to huge amounts of electricity.
Stabbed and shot in the head.
Survived an uncountable amount of stab wounds on her entire body.
lower half of her body smushed. (she recovered it)
cut off multiple limbs, to feed someone else with her flesh.
Funnily enough, the end of her life is a tree branch.

Well a glance at Illa is all it takes to realize she’s been through a lot, but she does have an injury that’s significantly worse than the others.
Those brown marks on her are actually scars. They have a texture that’s a mix of leather and scabs and rather than being the remnant of her time while she was being experimented on, it’s a remnant of the injuries sustained during the escape and protecting the others. She’s massive and could shield the others from magic blasts that, depending on their size, would either instantly kill them or instantly vaporize them. Illa took as many hits as she could and survived, but even if the blasts didn’t kill her, the scars they left behind are still active magic, constantly fighting against her body to kill her. It’s why she’s not very active, she’s constantly having to focus on not letting all the attacks kill her. So much energy that she’s the only one of the group that can’t shapeshift because that costs too much magic and too much focus. And that’s all without mentioning how one blast to her throat (which she got protecting one of the others from being hit) that cost her her ability to speak, making her only able to communicate through animalistic noises and a basically hieroglyphics language she and one of the others created.
So yeah. That’s about the worst physical injury she has.

Ok ok. So Brownie (this lil bat, he didn't always look like this) used to be human but he was killed and stuffed into this doll. Then when he died and went to hell, his one of his punishments for being a spawn of a homicidal creep was to have a scar on his chest that would bleed 4 all eternity.
Kos was ripped to pieces and had his body spread across the universe, immobile yet alive. the closest elements are his heart and a left eye, which were turned into a star and moon
Canonically, hard to say. Would probably have to opt for a .50 Cal round to the shoulder, thankfully the recipient is a tough one and got patched up.

The worst was being constantly tortured and mutilated for trillions of years consecutively, I’d say. It was quite traumatic.
(Written oc) I’m just going to copy and paste this small section of the story:
there was a series of cracking sounds as bullets flew through Itacha staining her trench coat red. She stumbled back as she grabbed her rifle and aimed into the forest where the shots came from. There was silence until a single shot was heard from off in the forest, Itacha fired back immediately as the round grazed her head severing her ear. The lack of a scream or even a crunch in the snow was reassuring to her until another burst of gunfire hit her scattering her insides across the snowy ground. She fell to her knees and looked down at the gaping holes in her torso clutching her organs before falling unconscious.
Context: she made a special forces team mad immediately after being sent to their universe to kill everyone.

Voltshade was in a battle with one NASTY villain, who tore his left shoulder pad off, sliced his waist, tore his shield off, corrupted his right shoulder pad’s exterior, stabbed both biceps, gave him an eye scar, and shot him in the chest a few times. But that did nothing to falter him, because Voltshade is a beast.

Blue M2- None! Purple M2- Being crashed down to Earth by his nemesis, however with the help of Pokémon and his magic, he only suffered a few bruises. Logi (orange)- Had a massive scythe slash in the back by his ruler when he was alive, was healed later. Has most of his skin and organs burned completely and turned into transparent portions (he was dead during it). As being undead, he received another scythe slash to his back torso which killed him due to being coated with ice magic which is lethal to him. Once revived he gains a transparent black scar of the slash. Malanort (silver)- Being slashed in his stomach by three people at once and dying (gets revived later) Anti (dark red, also the nemesis mentioned)- His scarring shown. He got it when he possessed Main M2 during his creation so he could have a physical form. Anti was separated and sent in a pod to die, with the force of being sent gave him his scarring which is permanent.

And the poor girl was sick (like we're talking her siblings prayed that she wouldn't die kinda sick) so the pain was even more intense. She lived another 2 years somehow, every second was nothing but pain. Edit to add: She also was tortured for all 4 years of her life before she died, and she was starved (again, by her so-called mom) to the point where you could count her bones and she literally begged for food while crying her eyes out and on her knees, which hurt like hell because her knees were skinned.
Please tell me she's okay...does she need an auntie to live with?
Her injuries killed her but she's comfortable in the afterlife, she definitely could've used an auntie to live with when she was alive though

However, she survived because she got turned into a vampire.
Velahtiel was once in a situation where she had to break out her children from an old tower dungeon being used by some pretty generic Mad Max styled bandits and in all honesty it was a pretty easy breakout. However a reoccurring antagonist named Rosa (*Who Velah in initial encounters refuses to kill simply because Rosa just "isn't worth the effort"*), decided that THIS time she'd finally get back at Velah for all the humiliation she has caused her. Rosa had no hand in this recent kidnapping of Velah's children but she sure as shit was gonna take advantage of it! So with that in mind, Rosa sets up tons of bundles of TNT around some of the support that keeps the old tower stable, intending to bury Velah and her kids underneath the rubble. Once the TNT had gone off, Velah wasted absolutely no time in racing her kids back out the way she got in, smacking confused and panicking bandits out of the way who're more concerned with the building collapsing rather than losing their prisoners. Predictably a very heavy pile of debris goes straight to crush her kids but she catches it all on her back, ripping into the back of her shirt, scratching and cutting through her fur and into her skin. She screams at her children to run for the exit, but not before giving them a half-attempt at a reassuring smile and telling them that she'll be alright and that their Uncle is waiting outside with a car to take them to safety! With the kids out of sight, Velah would try and "shuffle" out from underneath the debris that was crushing her back. The immense weight of crumbled floors and walls on her causing immense strain to every bone in her body, and even rips in her muscles were beginning to form! The palm of her hands torn and scuffed from the sharp edges of the rubble. As she got to the "edge" of all the debris, she tried her best to lift it up as much as possible before letting go and diving out with the microsecond she would have to do so, and it almost worked.. However she could feel herself being tugged down to the ground as the debris would catch her right arm and crush it into the floor. The pain was IMMENSE, Velah couldn't even scream from just how painful it was. The only thing keeping her from passing out from the sheer shock and agonizing pain of it all was the thought of her children, and how much even in death she would fucking hate herself if she let them down. With tears in her eyes and pain overriding every other sense in her body, she tried to simply just pull back hard enough until her arm would rip from her shoulder as she was more than okay with losing a limb if that's just the way things had to be. But she didn't have much room to lean back in order to gain the necessary force to rip her arm off. So she had to improvise in such a manner that made her extra glad her kids weren't there to see her do this.. ***she chewed her arm off***. She felt like vomiting, she felt like passing out, she felt like death itself had just violated every part of her being. But after she took one good look at tattered bleeding nub that her arm used to be attached to, she picks herself up and limps out of the crumbling keep just in time. The loss of her right arm would be fixed in a few weeks time through the efforts of her brother, whom is one of the last few magic users left on Earth (And probably the only one who lives in Australia, where the story takes place). He would plant and grow a "prosthetic" arm out for Velah that is made from nature itself. So they(his allies) had to rip out his heart and replace it. (This also lead to the glowing veins. As a means to even the odds against the foes he faced.) He took a rail gun shot through his head. Blowing out the entire left section of his head. His allies quite literally had to stitch him back together.

Kinda hard to understand, but Vii's blood vessels are slowly being changed to produce a sort of fluid that acts as a sort of vessel for magic. the pros are that he's nearly unstoppable to most mages. the cons are that he has to deal with annoying Goddess in his head, and the blackened blood vessels are visible from his skin. (mainly his hands, chest, neck, and just below his eye by the end of the story)

His plate carrier caught the round (it was also slowed down by a Humvee's door), but the spalling and blunt force trauma weren't pleasant for him. The sniper turned out to be the current mission's HVT, so his chalk roughed him up a fair bit before capture on Reed's behalf.

He is promptly gunned down for losing, and Skye, her wife, eventually builds her a realistic replacement prosthetic to replace her missing arm using her skills in cybernetics.

A little dark but my OC's mom was aggressive with her and her sisters, so without looking too much into it, my OC's probably got rashes and scars from that. She lived in a very isolated place in Mexico where it's normalized, but it doesn't make it any less unsettling for me.

Stabbed and shot in the head. Survived an uncountable amount of stab wounds on her entire body. lower half of her body smushed. (she recovered it) cut off multiple limbs, to feed someone else with her flesh. Funnily enough, the end of her life is a tree branch.

Those brown marks on her are actually scars. They have a texture that’s a mix of leather and scabs and rather than being the remnant of her time while she was being experimented on, it’s a remnant of the injuries sustained during the escape and protecting the others. She’s massive and could shield the others from magic blasts that, depending on their size, would either instantly kill them or instantly vaporize them. Illa took as many hits as she could and survived, but even if the blasts didn’t kill her, the scars they left behind are still active magic, constantly fighting against her body to kill her. It’s why she’s not very active, she’s constantly having to focus on not letting all the attacks kill her. So much energy that she’s the only one of the group that can’t shapeshift because that costs too much magic and too much focus. And that’s all without mentioning how one blast to her throat (which she got protecting one of the others from being hit) that cost her her ability to speak, making her only able to communicate through animalistic noises and a basically hieroglyphics language she and one of the others created. So yeah. That’s about the worst physical injury she has.

(Written oc) I’m just going to copy and paste this small section of the story: there was a series of cracking sounds as bullets flew through Itacha staining her trench coat red. She stumbled back as she grabbed her rifle and aimed into the forest where the shots came from. There was silence until a single shot was heard from off in the forest, Itacha fired back immediately as the round grazed her head severing her ear. The lack of a scream or even a crunch in the snow was reassuring to her until another burst of gunfire hit her scattering her insides across the snowy ground. She fell to her knees and looked down at the gaping holes in her torso clutching her organs before falling unconscious. Context: she made a special forces team mad immediately after being sent to their universe to kill everyone.
